Onsite flu shots: No appointment needed
As part of our ongoing commitment to employee health and well-being, Avnet is providing free flu shots to all employees* at the following locations. No appointment is necessary. Just bring a completed consent form with you and arrive at a convenient time during your day.

If you are unable to attend any of these events, UnitedHealthcare members can receive a flu shot for free at most retail pharmacies or convenience care clinics. Click here to find a location near you!

Mobile Onsite Mammography coming soon!
Avnet is teaming up with Mobile Onsite Mammography (MOM) to make it easier than ever to receive your annual mammogram. As early detection is the best defense we have against breast cancer, MOM’s professional staff will be at Avnet’s Phoenix facilities using the latest mammography technology for your safety and comfort. Spouses are welcome to make an appointment.

Call MOM at (480) 967-3767 to make an appointment and verify your insurance (MOM is a UnitedHealthcare Choice Plus network provider). Please have your insurance card available.
- MOM offers full field digital mammography and CAD computerized reading.
- $235.00 per exam for non-insured patients. Insurance claims will be submitted for all patients with health insurance. Patients need to be 35+
Employees enrolled in an Avnet medical plan can earn fun gift cards through our Live Well Rewards program. Earn $100 when you complete a preventive service (mammogram, exam, etc.) and submit your health survey!
IMPORTANT NOTE: It is your responsibility to consult your individual health plan for coverage and limitations as well as pay for any charges that are not covered. Patients under 35 years old must provide a doctor’s referral. Baseline mammograms start at age 35-39 (unless family history or symptomatic), and Avnet’s UnitedHealthcare plans cover preventive mammograms every year.
